1. Send the following information to [email protected].
- Official organization name, address (bill to and ship to if different), main phone number
- Main point of contact phone and email
- Copy of W-9
- Signed copy of the Texas Sales and Use Tax Exemption form
2. Send the initial Purchase Order directly to [email protected]; any subsequent orders may be placed by calling or emailing our Customer Service team, or entering it on our web portal via EDI. Sending the first time order to Eric will help alleviate any issues for the PDC internal teams.

At this time, PDC has an inventory of Generation I Texas Wristbands with part number 5070-35-TXM-SI; at a price of $45.48 per box of 500 bands. These would be available to ship immediately and are the bands that we've been using since the inception of this program a few years ago. At this time, the minimum order quantity on these bands is 2 boxes.
Generation II of the Texas EMS Wristband is explained in the graphic below, with part number 5070-35-TXSM; at a price of $45.48 per box of 500 bands. These bands are custom made to order and have a lead time that is still being determined---but that lead time will not be less than 2-3 weeks at this point. At this time, the minimum order quantity on these bands is 5 boxes.
